Pizzettas with Hand Pulled Mozzarella, Heirloom Tomatoes and Basil


Serves 10 people

Ingredients

For the Dough
2 teaspoons dry active yeast
1 cup lukewarm water
3 cups bread flour
2 tablespoons bread flour
2 teaspoons salt
3 tablespoons olive oil
1 teaspoon honey

For the Mozzarella
1/2 pound mozzarella curd
1/4 cup salt

For the Marinara Sauce
4 cups canned San Marzano plum tomatoes or canned Italian tomatoes
1/4 cup extra virgin olive oil
4 cloves garlic
1/2 bunch of fresh oregano
salt and pepper

Additional topping ideas
Heirloom tomatoes, basil, shaved asiago, roasted garlic, roasted red peppers, caramelized onions, sliced potatoes, fried eggplant, mushrooms.

To garnish
fresh basil, extra virgin olive oil, sea salt.

Instructions

For the Dough
Combine the yeast, water and honey in a small bowl. Let sit for 3 minutes. Add the olive oil, flour and salt. Mix to form a dough. Remove from bowl and knead on a clean work surface for ten minutes. Cover with a warm, damp towel and rest for one hour in a warm and humid part of the kitchen to allow dough to rise.

Punch down dough and transfer to a lightly floured surface. Cut into 10 even sized pieces. Roll into balls and pinch the seams to seal and place the seam side down. Cover with a damp cloth and let rest for 40 minutes before rolling out.

For the Mozzarella
In a 4-quart pot, bring 2 quarts water to a boil, then turn off the heat. Pour in 1/2 cup of salt and let it dissolve.

Slice the mozzarella curd as thinly as possible and place a big handful in a large mixing bowl. Pour the hot salted water over the curd until it covers it by about an inch. Let it sit for a minute before you go into action.

With a wooden spoon, begin moving the curd around until it begins to melt. Carefully pour off the hot water. Now, begin lifting and gently pulling the curd with your hands to remove any fibers or bumps. Stretch and pull the cheese back and forth until it becomes smooth and elastic. Before it has a chance to cool, roll it into a ball and squeeze it through your hands to form a smooth sphere.

For the Marinara Sauce
Poach the garlic cloves in the olive oil over low heat with a pinch of salt and pepper until the garlic begins to brown. Add the tomatoes and 1?2 of the oregano. Simmer for about one hour, adding a few tablespoons of water at a time to prevent the sauce from drying out. Continue to stir as the sauce simmers. Remove from heat and cool to room temperature. Process in blender until smooth. Chop the remaining oregano and add to the sauce. Season with salt and pepper.

To Make the Pizzettas
Roll out each dough ball to 1?4 inch thick circle, using flour to prevent sticking. Use your hands to stretch the dough. Sprinkle semolina flour on pizza peel (this prevents dough from sticking) and build pizzettas. Brush the edges of the dough with a little olive oil and spread the sauce on the dough. Top with all toppings. Slide onto stone and bake until the pizza is golden and crispy.

To Serve
Cut into pieces and finish with fresh basil, olive oil, sea salt and cracked black pepper.

Recipe: Steak au Poivre with Whipped Potatoes and Heirloom Carrots

Ingredients

For the Steak
3 pounds strip steak
? cup black peppercorns
? pound butter
2 tablespoons vegetable oil
salt

For the Sauce au Poivre
? cup shallots, minced
1 tablespoon mustard
? cup cognac
2 cups veal stock
2 tablespoons butter
3 tablespoons cream

For the Carrots and Beans
2 ? pounds heirloom carrots or regular carrots sliced in ? to ? inch pieces
or green beans, stems removed and washed
salt and pepper

For the Whipped Potatoes
3 pounds potatoes, boiled and peeled
? pound butter
2 cups milk
salt and pepper

Special Equipment

Heavy skillet
Pastry bag
Kitchen aid mixer with whip

Instructions

For the Steak
Cut the meat into 10 equal portions. Work the butter with a paddle or wooden spoon to soften. Crush the peppercorns in a mortar and pestle. Season the beef with salt. Spread one side of each steak with the softened butter. Coat the buttered side of the steaks with the cracked black pepper and chill.

Heat the oil in a heavy skillet and sear the pepper side of the steaks. Flip over and cook to medium/rare. Remove from pan and keep in a warm place while you prepare the sauce.

For the Sauce au Poivre
Add the shallots to the pan and cook over medium heat, stirring with a wooden spoon. Cook for a few minutes and then carefully add the cognac. Flambe the cognac and cook for a minute, then add the stock. Reduce over medium heat for 3 minutes. Stir in the mustard and mix to dissolve. Stir in the cream. Scald, then remove from heat and whisk in the butter. Season with salt and pepper.

For the Carrots and Beans
Steam or boil until al dente. Season with salt and pepper.

For the Whipped Potatoes
Boil the milk with the butter, salt and pepper. Combine with hot, peeled, boiled potatoes in the bowl of a stand mixer. Mix by hand to dissolve liquid into potatoes. With the whisk attachment, beat the potatoes (starting on low speed and accelerating slowly to high speed) for 45 seconds. Cover and keep warm.

To Serve
Spoon a scoop of whipped potatoes onto a plate and set the steak and veggies next to the potatoes. Sauce the steak and garnish with parsley.

THE SCHOOL OF PRACTICE

During and prior to the Civil War, musical training occurred at the "School of
Practice for U.S.A. Field Musicians" at Governor's Island, New York. The
earliest reference about the school is found in a book "Ten Years in the Ranks,
U.S. Army" written by a young soldier age 12, Augustus Meyers. He wrote about
his experiences at the school.

The living quarters were sparse, consisting of double bunk beds with insufficient
space for comfort or convenience. The beds were large sacks stuffed with straw.
The meals consisted of boiled salt pork and beef, rice soup, bread, potatoes, bean
soup, and coffee.

The daily duties began reveille with the fife and drums performing at the official
entrance to Governor's Island. At 0800, the guard mount ceremony commenced
followed by a period on uniform and equipment maintenance. School started at
0900 till 1100 followed by musical training from 1100 to 1200 and 1400 to 1600.
The young drummers and fifers performed at retreat. This schedule occurred
every day except Saturday when all instruction ended at 1200. In addition to
board, lodging and musical training, the boys received $7.00 a month.

The School of Practice studied from “The Drummers and Fife Guide” by George
G. Bruce. A board of musicians assembled by the War Department adopted this
book as the official text for the school. This manual was used until the end of the
Civil War.

Seven years later in 1869, a board of appointed officers investigated the system of
training field musicians. The board approved a method book called "Strube's
Drum and Fife Instructor" by Gardiner A. Strube.

THE CIVIL WAR

Five weeks after Abraham Lincoln's inauguration, Confederate soldiers fired on
Fort Sumter. The 1st Regiment of Artillery Band was present during the
bombardment and surrender. This band was also known as Chandler's Band of
Portland, Maine a civilian band volunteering its' service to the regiment.

The first band to suffer casualties during the Civil War was the 6th Massachusetts
Regiment Band. On April 19, 1861, the band arrived by train in Baltimore, MD.
As the band left the station a mob marching through the street attacked the band.
The band fled, abandoning all equipment, as local Union sympathizers took
bandmembers into their houses. The band suffered 4 deaths and 30 injured
personnel.

As the war began, the military was as divided as the country. Many of the Army's
finest officers resigned their commissions, returned home, and joined the
Confederacy. Enlisted personnel from the southern states deserted the Union
Army to fight for the South.

Faced with severe personnel shortages, President Lincoln called for 75,000
volunteers to serve three-month enlistments. Three weeks later, he realized that
the shortages would continue and called for 40,000 three-year enlistees and 40
additional regiments to be recruited, organized, and equipped by the states.

The Federal government allowed the states to establish their own recruiting and
organizational policies. Many volunteer regiments recruited bands. Civilian
bands with such famous conductors as Patrick Gilmore (whose band served with
the Massachusetts 24th Volunteer Regiment) volunteered their services to Union
regiments. Band recruiting was so successful that, by the end of 1861, the Union
Army had 618 bands and more than 28,000 musicians.

On July 22, 1861, Congress passed "An Act to Authorize the Employment of
Volunteers to aid in Enforcing Laws and Protecting Public Property." Section 2
authorized each Regular Army regiment of infantry two principal musicians per
company and 24 musicians for a band. Each cavalry and artillery regiment was
authorized two musicians per company or battery. Each Artillery Band was
permitted 24 musicians and each Cavalry Band was permitted 16 musicians. The
Army was following the same practice as Weiprecht in Germany with distinct
bands for each branch.

In response to a Congressional inquiry, the Paymaster General of the Army
reported the following pay scales for musicians:
First-Class $ 34.00 per month
Second-Class $ 20.00 per month
Third-Class $ 17.00 per month
Drum Major $105.50 per month

The federal government assumed the cost of volunteer regiments during the Civil
War. With the increase of the number of regiments, some members of Congress
became cost conscious. The cost of maintaining bands for all regiments was a
burden Congressmen did not want to bear. On January 31, 1862, Congress asked
the Secretary of War, Simon Camero, to evaluate the cost of each band and could
bands be dispensed without injury to the public service.

The Secretary of War reported the average cost of maintaining an artillery or
cavalry band was $9,161.30 and the cost of maintaining the larger infantry band
was $13,139.40. It was also reported 26 of 30 Regular Army regiments and 213
of 465 volunteer regiments had bands. The

A Manual of Simulation in Healthcare






Medical simulation is a relatively new science that is achieving respectability among healthcare educators worldwide. Simulation and skills centres have become established to integrate simulation into mainstream education in all medical, nursing, and paramedical fields. Borrowing from the experience and methodologies of industries that are using simulation, medical educators are grappling with the problem of rapidly acquiring the skills and techniques required to implement simulation programmes into established curricula. This book assists both novice and experienced workers in the field to learn from established practitioners in medical simulation. Simulation has been used to enhance the educational experience in a diverse range of fields; therefore a wide variety of disciplines are represented.

The book begins with a section on the logistics of establishing a simulation and skills centre and the inherent problems with funding, equipment, staffing and course development, and promotion. Section two deals with simulators and related training devices that are required to equip a stand-alone or institution-based centre. The features, strengths, and weaknesses of training devices are presented to help the reader find the appropriate simulator to fulfil their training requirements. There is a guide to producing scenarios and medical props that can enhance the training experience. The third section covers adult education and it reviews the steps required to develop courses that comply with 'best practice' in medical education. Teaching skills, facilitating problem-based learning groups and debriefing techniques are especially important to multidisciplinary skills centres that find themselves becoming a centre for medical education. The manual concludes with guides for the major specialties that use simulation, including military, paediatrics, CPR and medical response teams, obstetrics, and anesthesia.

Samuel Preston Moore, a native of Charleston, South Carolina graduated from the Medical College of the State of South Carolina in 1834 and quickly became assistant surgeon for the United States Army in 1835. This position required service in several frontier regions of the country, including Missouri, Kansas, Florida, and the Texas-Mexico border. While serving in the Mexican War (1846-48), Moore met the future President of the Confederacy, Jefferson Davis, who was quite impressed with his organizational and disciplinary abilities (Rutkow, Manual of milit. surg., vi). Moore was promoted to surgeon in 1849 and remained in this position through the 1850s. However, like many Southern officers in the United States Army, he was in crisis at the brink of the Civil War. When his home state of South Carolina seceded, he resigned his post in the U.S. army and moved to Arkansas to open a private practice and to avoid fighting against a country he had devoted so much of his life to. However, he began receiving personal requests from Jefferson Davis to join the Confederate army. Davis’ descriptions of the army’s unfortunate military situation and the lack of trained medical men eventually persuaded Moore to become surgeon-general in 1861, a position he would hold for the duration of the war (Rutkow, Manual of milit. surg., vii).

When Moore joined the Confederate army, they were already facing many medical-related difficulties, such as shortages of medicines, supplies, and equipment caused by the Union blockade of Southern ports and a lack of trained surgeons. Moore addressed all of these problems. He introduced a new type of large, one-story pavilion hospital and arranged for their construction throughout the South. He established an effective army hospital and field ambulance corps. Moore is also remembered for his resourcefulness in supplying the army with much-needed drugs by creating laboratories to prepare medications from indigenous Southern plants (Assoc. of the Med. Officers 3; Kelly & Burrage 865). In one issue of the Confederate States medical and surgical Journal (January 1864 – February 1865), a journal Moore started and directed, there is an article by him discussing the medicinal properties of Southern plants and criticizing the American Medical Association for not demanding that the blockade be lifted for medicines (Freemon 98-99). This article, “Indigenous remedies of the South,” is held at the Reynolds Historical Library within a bound volume of the journal from 1864-65.

Moore also instated measures to improve the quality and education of the Confederate army doctors. He set up an examination system that weeded out untrained doctors. If an examinee failed, he could be redeemed by serving as a hospital attendant and later retaking the exam (Freemon 40). In this way doctors who needed improvement were identified and trained. Also, Moore worked to increase the professional and technical knowledge of Confederate doctors by forming a professional organization, the Association of Army and Navy Surgeons of the Confederate States, founding the journal mentioned above, and compiling A manual of military surgery (1863). This manual is a collection of papers by surgeons which provides exact instructions (with drawings) for performing operations. It was intended for use by inexperienced surgeons in the army (Freemon 98). The Reynolds Library has a first edition of this 1863 publication.
